Build capacity and community resilience and tackle health inequalities using a place-based approach and provide support for newly emerging District Partnerships as appropriate

Provision of Public Health expertise to local Community Partnerships and District Partnerships where appropriate to support the development of locality planning in each CPP.

- Support the following where applicable: Place Planning; District Partnerships; Community Action Groups and Service redesign.

- Each locality plan identifies key performance measures against agreed partnership activities.
