Promote Social Prescribing as a pathway to overall health and wellbeing

Objective(s)

Manage the commissioning and development of the Community Link Worker Service.

Activities

  • Lead the development, support and extension of the delivery of the CLW service in Primary Care.
  • Support the evaluation of the CLW service in remote and rural areas of Highland in partnership with the University of the Highlands and Islands.
  • Scope and develop a Highland directory of services to support the CLW service.

  • Yearly update to NHS Highland Board (Social Mitigation Plan)

Support Financial Inclusion work.

  • Develop a financial inclusion service in remote and rural GP practices in Highland in partnership with Highland Council.

  • Overall work reported to Improvement Services by Highland Council.

  • 10 referrals per quarter (Improvement Service).
